Longevity360 Membership Terms & Conditions



Effective Date: 4/17/2026

This Membership Agreement (“Agreement”) governs participation in Longevity360 membership programs. By enrolling, you (“Member”) agree to the following terms with Longevity360 (“Practice”).


1. Membership Overview

Longevity360 memberships are designed to provide Members with ongoing access to aesthetic, wellness, and longevity services, including exclusive pricing, priority scheduling, and other benefits.

Memberships are non-transferable and intended solely for the enrolled Member.


2. Enrollment & Eligibility

  • Members must be 18 years or older 
  • All services are subject to clinical evaluation and approval 
  • Enrollment does not guarantee eligibility for any specific treatment 

Longevity360 reserves the right to refuse or discontinue services based on medical appropriateness or safety concerns.


3. Membership Fees & Billing

  • Membership fees are billed on a recurring monthly basis 
  • Payment is automatically charged to the card on file 
  • By enrolling, you authorize Longevity360 to process recurring payments 

Billing Terms:

  • Charges occur on the same date each billing cycle 
  • Failed payments may result in suspension of benefits 
  • Member is responsible for updating payment information 


4. Minimum Commitment 

Membership is month-to-month and may be canceled with appropriate notice.

5. Cancellation & Termination

  • Membership cancellation requires 30 days’ written notice 
  • No refunds will be issued for partial billing cycles 
  • Benefits remain active until the end of the billing period 

Longevity360 reserves the right to terminate membership for:

  • Violation of policies 
  • Inappropriate behavior 
  • Non-payment 


6. Membership Benefits

Benefits vary by membership tier and may include:

  • Discounted pricing on services 
  • Priority scheduling 
  • Access to exclusive treatments or events 

Important Conditions:

  • Benefits have no cash value 
  • Unused benefits do not roll over unless explicitly stated 
  • Benefits cannot be exchanged or substituted 


7. Service Usage & Limitations

  • Services are subject to availability and scheduling 
  • Treatments must be performed within professional guidelines 
  • Longevity360 may adjust treatment plans based on clinical judgment 


8. Refund Policy

  • Membership fees are non-refundable 
  • Services rendered are non-refundable 
  • Prepaid or unused benefits may be converted to account credit at the discretion of Longevity360 


9. Appointment Policy

  • A minimum of 24 hours’ notice is required for cancellations or rescheduling 
  • Late cancellations or no-shows may result in: 
  • Loss of scheduled service 
  • Deduction of benefit 
  • Fee charged to card on file 


10. Medical Disclaimer

All treatments are elective and results vary by individual.

Longevity360 does not guarantee specific outcomes. Treatment recommendations are based on professional judgment and may change over time.


11. Health Disclosure Requirement

Members are responsible for:

  • Providing accurate and complete medical history 
  • Updating any changes in health status 
  • Following pre- and post-treatment instructions 

Failure to do so may affect outcomes and eligibility for services.


12. Liability Waiver

Longevity360 is not responsible for:

  • Adverse outcomes resulting from failure to follow instructions 
  • Incomplete or inaccurate medical disclosures 
  • Treatments performed outside of Longevity360 


13. Non-Transferability

Memberships, services, and benefits:

  • Cannot be transferred to another person 
  • Cannot be sold or shared 
  • Cannot be redeemed for cash 


14. Promotions & Discounts

  • Membership benefits cannot be combined with other promotions unless stated 
  • Special offers may be time-limited and subject to additional terms 


15. Changes to Membership Program

Longevity360 reserves the right to:

  • Modify membership pricing, structure, or benefits 
  • Update policies and terms 

Members will receive reasonable notice of material changes.


16. Payment Disputes & Chargebacks

By enrolling, the Member agrees to contact Longevity360 directly to resolve billing concerns prior to initiating a dispute or chargeback.

Unauthorized chargebacks may result in:

  • Immediate termination of membership 
  • Suspension of services 
  • Additional administrative fees 


17. Privacy & Confidentiality

All personal and medical information is handled in accordance with applicable privacy laws and internal policies.


18. Governing Law

This Agreement shall be governed by the laws of the State of Delaware.


19. Acknowledgment

By enrolling in a Longevity360 membership, the Member acknowledges that they have read, understood, and agreed to these Terms & Conditions.
